After many successful sessions (my system has been up for 35 days), I
did a routine logout from my X session.

The X server did not restart, and I found
 xdm error (pid 26044): Unknown session exit code 2816 from process 25
in xdm-errors.

My /etc/inittab contains
 xd:3:respawn:/usr/bin/X11/xdm -nodaemon
and as you can see below I am in runlevel 3 with an xdm running.  I
don't know why xdm didn't restart properly.

I sent xdm (process 26044) a SIGTERM and init started another one; all
is now running correctly.
