I experienced similar problems when I installed debian from the 0.93R6
prerelease disk set early in August. After autoprobing the Buslogic controller
the machine silently hanged. I succeeded only after making my own boot disk
with a tailored kernel.

My hardware configuration is similar to yours, except that the single IDE disk
is enabled. This occured with 2 different revisions of the 1542B board.

Looks like the Buslogic autoprobing is disturbing the 1542B setup.

I have reproduced this problem with:
chiark:~> ll /usr/src/debian-archive/private/project/pre-release/1440_boot_floppy.gz
-r--r--r-- 1 ian ian 686439 Sep 16 21:59 /usr/src/debian-archive/private/project/pre-release/1440_boot_floppy.gz

I didn't write down all the numbers and things again; they looked
roughly the same as the last time (quoted below).

Subject: lilo-1.3.11 bootdisk 25th July fails

Package: (bootdisk)
Version: lilo-1.3.11-bootdisk-24-Jul-95

(This message was, I think, lost the first time around. If this is
the second copy you see let me know ASAP at iwj10@thor.cam.ac.uk, as
it means that some other message was lost.)

This bootdisk gets very badly confused while trying to talk to my SCSI
controller. The messages I see are (typed in from a piece of paper,
so not necessarily completely accurate; `...' indicates something that
didn't seem relevant which I didn't write down):

hda: Conner ....
ide0: primary interface on irq 14
Floppy drive(s): fd0 is 1.44M
FDC 0 is a 8727A
aha152x: porttest failed
BusLogic SCSI: buslogic_out: failed (4): BusLogic SCSI: buslogic_stat
 status=39 intrflags=84
BusLogic SCSI: setup_mailboxes failed
Configuring Adaptec at IO:330, IRQ 11, DMA priority 5
eata_dma ... skipping ...
scsi0 : Adaptec 1542
scsi : 1 host
scsi : aborting command due to timeout : pid 0 scsi 0, channel 0, id
 0, lun 0, inquiry 00 00 00 ff 00
scsi : aborting command due to timeout : pid 0 scsi 0, channel 0, id
 0, lun 0, inquiry 00 00 00 ff 00

and it just goes downhill from there. The SCSI disk light comes on
and stays on sometime around the aha142x or BusLogic probe.

The message about my Adaptec, a 1542B, looks right - it's the same one
as I normally see. My /proc/interrupts, /proc/ioports and /proc/dma
are below.

As a complication, the single IDE hard disk I have is not in the
CMOS. I don't think this is relevant, though.
