                     INSPECTION CHECKLIST


           ELECTRICAL WIRING, FIXTURES AND CONTROLS

[ ]  Are your work place electricians familiar with the Cal/OSHA
     Electrical Safety Orders?

[ ]  Are all electrical cords strung so they do not hang on
     pipes, nails, hooks, etc?

[ ]  Is all conduit, BX cable, etc., properly attached to all
     supports and tightly connected to junction and outlet boxes?

[ ]  Is there any evidence of fraying on any electrical cords?

[ ]  Are rubber cords kept free of grease, oil and chemicals?

[ ]  Are metallic cable and conduit systems properly grounded?

[ ]  Are portable electrical tools and appliances grounded or of
     the double insulated type?

[ ]  Are all ground connections clean and tight?

[ ]  Are fuses and circuit breakers the right type and size for
     the load on each circuit?

[ ]  Do switches show evidence of overheating?

[ ]  Are switches mounted in clean, tightly closed metal boxes?

[ ]  Are motors clean and kept free of excessive grease and oil?

[ ]  Are motors properly maintained and provided with adequate
     overcurrent protection?

[ ]  Are bearings in good condition?

[ ]  Are portable lights equipped with proper guards?

[ ]  Are all lamps kept free of combustible materials?

[ ]  Is your electrical system checked periodically by a
     competent person to assure compliance with all safety
     requirements?

                       EXITS AND ACCESS

[ ]  Are all exits visible and unobstructed?

[ ]  Are all exits marked with a readily visible sign that is
     properly illuminated?

[ ]  Are there sufficient exits to ensure prompt escape in case
     of emergency?

[ ]  Are controls in place for areas requiring limited occupancy?

[ ]  Do you take special precautions to protect employees during
     construction and repair operations?


                       FIRE PROTECTION

[ ]  Are portable fire extinguishers provided in adequate number
     and type?

[ ]  Are fire extinguishers inspected monthly for general
     condition and operability and is a proper notation made on
     the inspection tag?

[ ]  Are fire extinguishers recharged regularly and a notation
     made on the inspection tag?

[ ]  Are fire extinguishers mounted in readily accessible
     locations?

[ ]  If you have interior stand pipes and valves, are these
     inspected regularly?

[ ]  If you have a fire alarm system, is it tested at least
     annually?

[ ]  Are plant employees periodically instructed in the use of
     extinguishers and fire protection procedures?

[ ]  Are fire doors and shutters in good operating condition?

[ ]  Is your local fire department well acquainted with your
     plant, location, and specific hazards?


                     AUTOMATIC SPRINKLERS

[ ]  Are water control valves, air and water pressures checked
     weekly?

[ ]  Is maintenance of the system assigned to responsible persons
     or a sprinkler contractor?

[ ]  Are sprinkler heads protected by metal guards where exposed
     to mechanical damage?

[ ]  Is proper minimum clearance maintained below sprinkler
     heads?


                          MATERIALS

[ ]  Are approved safety cans or other acceptable containers used
     for handling and dispensing flammable liquids?

[ ]  Do you have a NO SMOKING rule enforced in areas involving
     storage and use of hazardous materials?

[ ]  Are NO SMOKING signs posted where needed?


                   FORK LIFT TRUCK OPERATIONS

[ ]  Are only trained personnel allowed to operate forklift
     trucks?

[ ]  Is overhead protection provided on high lift rider trucks?


          HOUSEKEEPING AND GENERAL WORK ENVIRONMENT

[ ]  Is smoking permitted in "safe areas" only?

[ ]  Are NO SMOKING signs prominently posted for areas containing
     combustibles and flammables?

[ ]  Are rubbish and litter disposed of daily?

[ ]  Do your toilet facilities meet the requirements of
     applicable sanitary codes?

[ ]  Are washing facilities provided?

[ ]  Are all areas adequately illuminated?

[ ]  Are stairways in good condition and standard railings
     provided for every flight having four or more risers?

[ ]  Are portable wood ladders and metal ladders adequate for
     their purpose, in good condition, and provided with secure
     footings?

                        LOADING DOCKS

[ ]  Are dockplates kept in serviceable condition and secured to
     prevent slipping?

[ ]  Do you have means to prevent car or truck movement when
     dockplates are in place?


                MEDICAL SERVICES AND FIRST-AID

[ ]  Are your first-aid supplies adequate for your work place?


              MANAGEMENT COMMITMENT AND PLANNING

[ ]  Established policy and objectives, communicated to
     employees.

[ ]  Responsibilities defined and authority assigned.

[ ]  Supervisor performance evaluated, and rewarded or corrected.

[ ]  Adequate company resources allocated for safety and health
     (in staff, equipment, safety promotion, etc.)

[ ]  Clear lines of communication for safety and health concerns.

[ ]  Management sets example on safe and healthful behavior.


                      HAZARD ASSESSMENT

[ ]  Completion of comprehensive safety and health surveys.

[ ]  Periodic self-inspections, conducted by qualified person.

[ ]  Employees know how and whom to notify, fear no reprisal, and
     receive timely and appropriate responses.

[ ]  Procedures to identify causes and needed corrective action.


                HAZARD CORRECTION AND CONTROL

[ ]  Engineering and PPE controls in place, as appropriate.

[ ]  Administrative controls, including safety and health rules
     and safe work procedures, established and implemented.

[ ]  Procedures for timely correction/control of newly-identified
     hazards.
[ ]  System for rule enforcement and discipline established and
     carried out.

[ ]  On-going monitoring and maintenance of equipment.

[ ]  Preparation with training and drills as needed, for
     emergencies requiring PPE, medical care, emergency egress,
     etc.

[ ]  Emergency telephone numbers, exit routes, etc., visible to
     all.


                  SAFETY AND HEALTH TRAINING

[ ]  Supervisors can explain rules, procedures and work practices
     for hazard control, and how they teach and enforce them.

[ ]  Employees can explain how and why to do their jobs safely.

[ ]  Training is reinforced or repeated as necessary.

[ ]  Employees use PPE when necessary.

[ ]  Employees know why, how, limits and maintenance of PPE.


              ENVIRONMENTAL HEALTH

[ ]  Is lighting adequate?  Can you increase the performance and
     efficiency of your employees by decreasing glare or
     increasing the quality of light?

[ ]  Are employees instructed on the correct procedures for
     lifting heavy objects?  Are materials placed, when possible,
     to minimize lifts from floor levels?
