The following chapter from the City of Los Angeles Safety Manual is
a draft.  It has been submitted so that others may utilize the
information in helping to meet the requirements of the Written
Injury and Illness Prevention Program required by Senate Bill 198
and the expanded General Industry Safety Order 3203.

                     SAFETY INSPECTIONS


GENERAL INFORMATION

This chapter establishes inspection and evaluation requirements and
procedures.  These inspections cover the conditions of buildings,
tools, machines, equipment, materials, procedures in operations and
processes, and the actions of persons.

The City's safety staff, including Safety Coordinators, safety
committees and members of the Occupational Safety and Industrial
Hygiene Offices, help to develop programs and aid in solving safety
and health related problems.  Facility inspections and job
observations produce meaningful recommendations.  Management's
corrective actions on those recommendations are an important part
of an effective safety program.

Safety inspections help to identify hazards and evaluate compliance
with safety program requirements.  They shall be conducted
according to a regularly scheduled plan.  Safety inspections may be
made by management, supervision or employees designated by super-
vision.  It has also been found that it is beneficial to have
safety committees make inspections.

In all cases, records of unsafe conditions shall be kept (see
Chapter 2, INJURY AND ACCIDENT PREVENTION PROGRAM).  Rec-
ommendations shall be made for the correction of unsafe conditions
or procedures and a date should be scheduled for the completion of
each correction.  The problem's resolution should be described in
a follow-up report.


SAFETY INSPECTIONS

Managers and supervisors are responsible for providing a safe
workplace and for their subordinates knowing and complying with
safety standards.  Thorough inspections help management determine
the condition of work areas, the degree of compliance with stan-
dards, and the safety of work practices.

A. INSPECTION REQUIREMENTS:

   1.   Supervisors are responsible for inspecting their
        respective City facilities on a regular schedule.

        a.   Facilities such as shops, construction sites and
             maintenance yards shall be inspected monthly or
             when conditions warrant more frequent inspections.

        b.   Facilities such as office spaces, libraries and
             storage rooms should be inspected every three
             months or when conditions warrant more frequent
             inspections.

   2.   Each supervisor should conduct periodic spot inspections
        in addition to the above requirements.  

B. SAFETY INSPECTION PROCEDURES:

   1.   To properly conduct safety inspections, management shall
        select personnel who are qualified in the identification
        of hazards, and knowledgeable regarding the Cal/OSHA
        regulations covering the subject workplace.

   2.   Inspections may be conducted with or without prior
        notice.

   3.   Inspectors shall talk with any employee who wants to
        discuss safety matters.  Supervisors shall give them the
        opportunity to identify unsafe working conditions or
        practices.

   4.   Inspection checklists should be used, especially for
        items common to several areas or facilities.  Checklists
        should be made available to the supervisors of the
        activities to be inspected.

   5.   Safety Inspection Reports: 

        a.   Inspection reports shall be in written format. 
             These reports shall include the following:

             (1)  The facility, activity, or work area
                  inspected.

             (2)  The date of the inspection.

             (3)  A description of any safety hazards noted.

             (4)  Descriptions of any unsafe work practices
                  observed.

             (5)  Causes of the deficiencies and hazards noted.

             (6)  Recommendations for corrective action.

             (7)  A proposed schedule for completing the correc-
                  tive actions.


        b.   Formal reports should be kept by the supervisor,
             with copies forwarded to the functional area
             manager of the activity inspected and to the
             departmental Safety Coordinator responsible for the
             safety program.  In addition, the Occupational
             Safety Office shall be sent a copy of the formal
             report.  Your department may have additional
             reporting requirements.

            c.    The Occupational Safety Office and/or Industrial
                  Hygiene staff may monitor for corrective action and
                  may conduct follow-up spot inspections to make sure
                  that such corrective actions are adequate.
