The following chapter from the City of Los Angeles Safety Manual is
a draft.  It has been submitted so that others may utilize the
information in helping to meet the requirements of the Written
Injury and Illness Prevention Program required by Senate Bill 198
and the expanded General Industry Safety Order 3203.

                                                        Chapter 5







RESPONSIBILITY


1.   Each employee must immediately report every work-related
     injury, no matter how slight, to supervision.  A work-related
     injury or illness is one which occurs as a result of a
     person's employment with the City.  If the injury is serious
     and no supervisor is present, get medical attention first. 
     Supervisors must report or log all work-related injuries.  If
     a supervisor believes an injury requires medical attention,
     the Personnel Department's Workers' Compensation Division must
     be contacted for instructions.

2.   The immediate supervisor must provide the employee with Form
     Gen. 66, EMPLOYEE'S CLAIM FOR WORKERS' COMPENSATION BENEFITS,
     within 24 hours of any injury claim.  This must be done even
     if the employee is not at work (see Chapter 6).

3.   Immediately after securing medical aid, or on the first
     business day after an evening or weekend injury, telephone the
     Workers' Compensation Division, at 485-5683, to report the
     injury.  (Hours 7:00 a.m. to 6:00 p.m.)

4.   For all injuries requiring medical treatment, make a written
     report of the injury on Form Gen. 166, EMPLOYER'S REPORT OF
     OCCUPATIONAL INJURY OR ILLNESS, and distribute the copies
     promptly (see Chapter 7).

5.   If assistance in obtaining treatment for injured employees is
     required during evenings or weekends, a Workers' Compensation
     representative may be reached through the City Hall, Chief
     Telephone Operator, 485-5500.

6.   The employee is responsible for notifying supervision whenever
     the employee is taken off duty or returned to work by the
     doctor.  The employee must get a "Return to Duty" slip after
     each visit to the doctor.  The supervisor is responsible for
     knowing the status of the injured employee.

7.   If the employee is unable to return to duty after receiving a
     "Return to Duty" slip, the employee must return to the doctor
     or call Workers' Compensation Division immediately for further
     directions.


PROCEDURE


Treatment and reporting of on-duty injuries to civilian employees.

I.  Serious Injuries

     A.   Within City limits:  Telephone the closest location
          listed below for a City ambulance or take the employee to
          the nearest emergency hospital if he can be safely moved.

     B.   Outside City limits:  Use the nearest available emergency
          hospital or ambulance service.

II.  Minor Injuries

     A.   On weekdays between 7:00 a.m. and 5:30 p.m., telephone
          the Workers' Compensation Division for instructions, 485-
          5683.

     B.   Issue form Gen. 83, "Non-Emergency Medical Order", and
          secure medical treatment as instructed by the Workers'
          Compensation Division.

     C.   On holidays, weekends, or evening hours, take the injured
          employee to the nearest 24-hour industrial facility or
          emergency hospital.  The Fire Department, at 483-6721,
          can provide the location of the nearest industrial
          facility or hospital.  A Workers' Compensation
          representative may be reached for help by calling 485-
          5500, if necessary.

III.  Deaths

     A.   If within City limits, telephone the Police Department,
          at 485-2681, or the nearest Police Station.

     B.   If outside the City limits but within Los Angeles County,
          telephone the nearest Sheriff's Office.

     C.   Notify the Workers' Compensation Division at 485-5683.

     D.   Notify the City Occupational Safety Office at 485-4691.
