The following chapter from the City of Los Angeles Safety Manual is
a draft.  It has been submitted so that others may utilize the
information in helping to meet the requirements of the Written
Injury and Illness Prevention Program required by Senate Bill 198
and the expanded General Industry Safety Order 3203.

HOW TO REPORT INJURIES TO:    CAL/OSHA, OCCUPATIONAL SAFETY OFFICE
                              WORKERS' COMPENSATION, CITY ATTORNEY


RESPONSIBILITY

     Every supervisor shall report immediately by telephone to the
     nearest district office of the California Division of Occupa-
     tional Safety and Health (DOSH) any serious injury, illness or
     death, of an employee occurring in a place of employment or in
     connection with any employment.

     IMMEDIATELY means as soon as possible but not longer than 24
     hours after the employer knows, or with diligent inquiry would
     have known, of the death, or serious injury or illness.

     SERIOUS INJURY OR ILLNESS is defined by the California Code of
     Regulations as "any injury or illnesses occurring in a place
     of employment or in connection with any employment which
     requires inpatient hospitalization for a period in excess of
     24 hours for other than medical observation or in which an
     employee suffers loss of any member of the body or any serious
     degree of permanent disfigurement."  Serious injury or illness
     does not include any injury, illness, or death caused by
     commission of a Penal Code Violation, except of the Penal Code
     Section 385, (which refers to the illegal operation of heavy
     equipment adjacent to high voltage wires), or a vehicle acci-
     dent on a public street or highway.  A death resulting from a
     highway vehicle accident need not be reported to DOSH, even
     though the death is work-related. 

     According to California Code of Regulations Section 342,
     "Whenever a state, county, or local fire or police agency is
     called to an accident involving an employee covered by this
     part in which a serious injury, or illness or death occurs,
     the nearest office of the Division of Occupational Safety and
     Health shall be notified by telephone immediately by the
     responding agency."  This report may be made by any person
     authorized by the employer, state, county, or local agencies
     making such report.

     Each employee must immediately report all injuries, no matter
     how slight, to supervision.  If the injury is serious and no
     supervisor is present, the employee should get medical
     attention first.

     California law requires an employer to report, within five
     days, every industrial injury or occupational disease which: 

          1.   results in lost time beyond the day of injury; or 
          2.   requires medical treatment other than first aid.  

     This is accomplished by completing Form Gen. 166, Employer's
     Report of Occupational Injury or Illness (see Chapter 7).  The
     Cal/OSHA Form 200 must also be updated (see Chapter 8).


NOTIFICATION PROCEDURE

     If a supervisor believes that an injury may require medical
     attention, the Personnel Department, Workers' Compensation
     Division, shall be contacted for instructions (see Chapter 5).
     

     In case of injury or death when a City vehicle is involved,
     the supervisor shall notify the City Attorney's Office (see
     Chapter 4).

     The Occupational Safety Office shall be notified immediately
     when any of the following occur:

          1.   Death or serious injury to a City employee while on
               duty.

          2.   Death or serious injury to any person which in-
               volves City facilities, employees or vehicles owned
               or rented by the City.

          3.   Any accident which indicates a real and apparent
               probability that a serious injury could have oc-
               curred.

     The safety engineering staff can be reached at 485-4691 or,
     after regular working hours, by calling the City Hall Chief
     Operator at 485-5500 and asking to speak to a Safety Engineer.
     

     In every case involving a death, serious injury or illness, a
     report shall be made immediately by the supervisor or
     department representative to the nearest district office of
     the California Division of Occupational Safety and Health by
     telephone.  Telephone numbers of all district offices are
     listed on the Cal/OSHA poster which should be displayed at
     each facility.  When making such a report, the reporting party
     shall include the following information, if available:

          1.   Time and date of the accident.

          2.   Employer's name, address and telephone number.

          3.   Name and job title, or badge number of the person
               reporting the accident.

          4.   Address of the accident site or event.

          5.   Name of the person to contact at the accident site.

          6.   Name and address of the injured employee(s).

          7.   Nature of the injury.

          8.   Location to which the injured employee(s) was
               (were) moved.

          9.   List and identity of other law enforcement agencies
               present at the accident site.

          10.  Description of the accident and whether the acci-
               dent scene has been altered.
