 AB    The ICD p24 antigen has been demonstrated to increase the sensitivity of
       p24 antigen testing in adults and children. The ICD p24 antigen has been
       associated with increasing Walter Reed Stages in adults. We sought to
       determine if there was an increased positivity of the ICD p24 antigen
       associated with CDC pediatric stages in HIV infected children and if
       anti-retroviral treatment was associated with decreased ICD p24 antigen
       positivity. 31 HIV infected children followed by the Military Pediatric
       HIV Consortium had 99 serum samples assayed over an 18 month time
       period. HIV infection was defined as at least two blood specimens
       positive by HIV DNA PCR or HIV co-culture. Sera was tested with the
       Coulter HIV-1 Ag Assay. Microtiter sera samples were acidified with
       glycine and neutralized with Tris, incubated overnight and detected in
       an antigen capture ELISA format. Statistical analysis was performed by
       Chi-Square. There were 99 positive samples and 12 negative samples
       yielding a sensitivity of 89.2%. There was a statistically significant
       association between ICD p24 antigen positivity and stage (P1
       asymptomatic vs P2 symptomatic) but not between P1 and P2A vs > P2A
       stages. This significance was also true for quantitative ICD p24
       antigen. There was a statistically significant association between p24
       antigen positivity and those patients on therapy. We have demonstrated
       that ICD p24 antigen positivity is associated with symptomatic pediatric
       CDC stages and that treatment did not negatively effect our qualitative
       ICD p24 antigen results.
