 AB    OBJECTIVE: To describe the epidemiology of perinatal HIV exposure and
       transmission in Australia. METHODS: A registry of all cases of perinatal
       exposure to HIV was established using data retrieved retrospectively to
       test for possible factors affecting transmission rate, maternal and
       infant variables were examined using Chi square. RESULTS: Between 1982
       and June 1993, 72 babies were reported as perinatally exposed to HIV
       infection. Six infants have indeterminate status. Of the 66 children for
       whom infection status could be determined, 22 (33%) have acquired HIV
       infection. Infants horn to mothers infected as the result of injecting
       drug use had a 47% transmission rate. Those infants born to mothers from
       Pattern II countries had a transmission rate of 83% (p = 0.02). Among 19
       infants known to have been breastfed, 10 are infected (52%). Among 29
       infants who were never breastfed, 7 are infected (24%) (p = 0.04). No
       other factors were noted. CONCLUSION: The perinatal HIV register appears
       informative and prospective data acquisition is advocated.
