AB In early 1993 an elderly Vietnamese lady presented to Fairfield Hospital
with skin lesions, fever and was found to have disseminated MAC and
cryptococcal meningitis. Subsequent tests revealed a profound
suppression of lymphocytes, with a CD4 count of 270 and CD8 count of 80.
Despite treatment and clinical improvement her lymphopaenia worsened
with counts of 150 (CD4) and 50 (CD8) respectively. All tests for HIV 1
and 2, HTLV 1 and 2 and malignancy have been negative. She now remains
well and has shown a progressive rise in lymphocyte counts.
Nevertheless, we believe that this lady meets the criteria for
idiopathic T helper lymphocytopenia as defined by the Centers for
Disease Control and we believe is the first such case description in
Australia. The diagnostic criteria and natural history are briefly
       discussed.
