AB Purpose: To study the contribution chest CT makes to the staging and
clinical management of non-Hodgkin's lymphoma at the time of initial
presentation. Materials and methods: retrospective review of chest CT
scans, chest radiographs and subsequent hospital course in 59 patients
(pts) with newly diagnosed non-Hodgkin's lymphoma (excluding HIV
positive pts). Results: 42 of the 59 pts (71.2%) had normal chest
radiographs. The accompanying CT scans showed the presence of undetected
axillary, mediastinal or hilar adenopathy, unsuspected pleural or
pericardial effusion or pulmonary infiltrate in 20 of these pts (47.0%).
This resulted in a change of their radiographic staging. Of the 17
remaining pts with an initially abnormal chest radiograph, the CT scans
confirmed abnormalities in all cases. In 11 of the 17 pts (64.7%) with
an abnormal chest radiograph, the CT demonstrated more intensive disease
and additional areas of involvement. Conclusion: CT proved to be of
great value in providing an accurate staging at the time of the initial
presentation in lymphoma especially when compared to the chest
radiograph. This accurate staging allows for the inclusion into the
proper treatment protocol as well as more accurate assessment of
       subsequent response.
