Convert MMD0 and MMD1 (OctaMED and MED) modules
to MID file format 1.

MUI and CLI only versions both in the archive!

Version 2.5b features automatic MIDI send optimization.
Look in the .guide file inside the archive for the
complete list of bug fix, optimizations and enhancements.
This is how it should have been from the start.
