        If you send a personal check, there may be a delay while waiting
        for the funds to clear, before your copy is sent to you.  Heck,
        I'll accept cash too, but I cannot be responsible if it gets lost.
        
        To use this as an ordering form, just print out a copy on a sheet
        of paper and then answer the questions.  Or just send me the
        necessary information with your check some other way.  Everything
        after the first two questions is optional.  First, I need to know
